Traverse Independence supports adults with physical disabilities and acquired brain injuries (ABI) to live independently in the community. As a centre of excellence, we empower and connect clients on their journey to independence.
This position includes reporting to the manager for our ABI Outreach Program. You will provide support to individuals in their homes or the community. You must have demonstrated experience in behaviour management skills and experience with people with Acquired Brain Injuries. Candidates must have proven organizational skills, interpersonal skills and communication skills (both written an oral), ability to work as a member of a multi-disciplinary team with a comprehensive understanding of Microsoft Office products.
Community Facilitator Requirements:
Education: Community College Diploma in Social Services, Health Care or related discipline.
Experience: Minimum of 3 years demonstrated experience in rehabilitation of ABI clients or developmental services.
Knowledge/Skills: Demonstrated experience in behaviour management skills & knowledge of community resources.
- Experience with people with disabilities resulting from an ABI.
- Applied application of computer hardware and software.
- Demonstrated organizational skills, interpersonal skills and communication skills both written an oral, ability to work as a member of a multidisciplinary team.
- Must have valid driver's license and access to reliable vehicle with proof of 1,000,000 liability insurance and drivers abstract acceptable to Traverse Independence.
