Position: Case Manager

Hours: Monday to Friday - days and evenings, 8 hour shifts (Hours subject to change based on operational needs)

This is what you’ll be doing:
The Case Manager will provide intense case management services to a caseload of severely and persistently mentally ill adults, some of whom may require to be on Community Treatment Orders and/or requiring long acting injections. To be successful in this position, you possess strong assessment skills, crisis management, advocacy skills, decision-making, effective coordination of care and computer skills combined with the ability to work as part of an interdisciplinary team.You need to have a good working knowledge of the Mental Health Act including Bill 68 amendments and related legislation (Substitute Decision and Health Care Consent Acts).
Your role will consist of acting as a liaison between your clients and the various health care providers/community agencies needed to achieve the client optimal health and independence. The goal is to develop and monitor a plan for clients across the psycho-social realm. Patients are seen in the community and the goal is to promote independence, wellness and rehabilitationwithin the framework of recovery. The incumbent will require a minimum of 2 yearscurrent experience with persons with amajor and persistent mental illness.

- Strong Case Management experience required.
- Current registration in good standing with the College of Nurses of Ontario (CNO) required
- CBT and/or DBT training.
- Experience with formal group therapy.
- Ability to work effectively and collaboratively within the interdisciplinary team.
- Experience in facilitating care for patients within the healthcare and community services.
- Extensive knowledge of psychiatric illness and treatment thereof including psychotropic medications.
- Strong knowledge of community resources; exemplary collegial relationships with all disciplines.
- Ability to complete a suicide risk assessment
- Ability to work effectively and compassionately with the most ill clients.
- Possession of a car and a valid driver's license are required.
- Satisfactory employment and attendance record
