About the Role:
- In a one-on-one case management setting you will assess customer’s base level of function, set recovery goals, design and implement a return to function/work plan and identify recovery milestones.
- Your natural empathy and well-developed communication skills will aid you in supporting your customers throughout their recovery journey, identifying their needs while building and maintaining relationships with a variety of stakeholder including health care professionals and others.
- The work week is Monday – Friday, 37.5 hours per week and is a permanent opportunity starting early March 2021
About you:
- Together with your education and experience in disability case management you have a genuine desire to build trusted relationships with all stakeholders and have outstanding listening skills.
- You take ownership and pride in putting customer’s concerns, preferences, values and needs at the centre of the recovery goals.
- You excel at document management, keeping all files up to date and recording all details pertinent to a customer’s case.
- You have a collaborative work style while managing a broad spectrum of cases, coordinating and managing multiple care and service providers involved in the customer’s recovery journey.
Position Requirements:
- Degree or Diploma in Kinesiology, Occupational Therapy, Physical Therapy or Disability Management
- Minimum 2 years recent professional experience in disability management or vocational rehabilitation or an equivalent combination of education and direct experience in vocational rehabilitation or disability case management
- Strong problem solving and conflict resolution skills
- Highly detail oriented and organized
- Able to handle multiple files and tasks on a daily basis
- Exceptional communication and listening skills
- Ability to work under pressure
- Second language is an asset – written and verbal
